How to Register

steth_book1The Practice has a designated practice area agreed with the Primary Care Trust (PCT)

A patient wishing to register will be asked for a passport and a current utility bill to verify address and personal details to ensure that they are eligible for NHS care and are living within our area.

If we are unable to register you, we will provide you with a reason in writing.

All new patients will be asked to see a member of staff for a basic health check. This involves completing a health questionnaire, recording of height, weight, blood pressure and a urine test to screen for diabetes and kidney disease. It is also an opportunity to discuss any medical or other problems.

The practice cannot guarantee the patient will always see the doctor of their choice but their preference will be recorded and taken into consideration when making an appointment.

You may print off a registration form, fill it out and then call us to make an appointment for your health check. Please bring all your paperwork with you on your first visit to the practice.
